Job summary

Qualifications

• Valid driver’s license with a satisfactory driving record.
• Minimum 2 years of experience supporting individuals with disabilities.
• Ability to lift or move 25+ pounds if required.

Responsibilities

• Support individuals to increase independence.
• Meet with waiver participants regularly.
• Document progress toward community integration.
• Monitor health and medical needs of individuals.

Education
