Job summary

Access: Supports for Living seeks a Community Habilitation Worker (CHW) to mentor individuals with developmental disabilities in community and home settings in Montgomery, NY. You will support personal growth, independence, and social integration while maintaining plan-based services.

The role requires valid driver’s license, reliable vehicle, and willingness to transport clients. Flexible schedules and travel to multiple locations are part of the position; bilingual skills are a plus.

Qualifications

• Experience mentoring and supporting individuals with developmental disabilities or related populations.
• Strong communication and interpersonal skills with a person-centered approach.
• Ability to document services accurately and follow service plans.

Responsibilities

• Provide mentoring and teaching to individuals to help achieve personal outcomes.
• Teach daily living skills such as budgeting, socialization, cooking, job search, and community integration.
• Work with individuals in home and community settings several times per week.
• Travel to assigned locations to deliver Community Habilitation services.
• Maintain timely documentation and report progress and barriers.
