Job summary

Resources for Human Development (RHD) is hiring a Community Inclusion Specialist in Medford, MA. The role supports adults with developmental disabilities through community-based activities, implementing ISPs, and facilitating meaningful programs to promote independence and community involvement.

  • High School Diploma or GED equivalent.
  • Minimum of three (3) years of experience supporting adults with developmental disabilities.
  • Valid driver's license and reliable personal vehicle.
  • Basic computer proficiency.
  • Strong communication and interpersonal skills.
  • Ability to lift up to 50 pounds.

Responsibilities

  • Support adults with developmental disabilities through community-based activities, outings, and individualized programming.
  • Implement ISPs and person-centered goals.
  • Facilitate meaningful recreational, educational, vocational, and social activities.
  • Promote independence, self-advocacy, and active participation in the community.
  • Provide transportation to community activities, events, and appointments.
  • Complete required documentation, progress notes, and activity records.
  • Manage outing budgets and activity-related expenses as needed.
  • Ensure participant health, safety, and well-being, including basic first aid when necessary.

What You'll Do

As a Community Inclusion Specialist, you will:

  • Support adults with developmental disabilities through community-based activities, outings, and individualized programming
  • Implement Individual Support Plans (ISPs) and person-centered goals
  • Facilitate meaningful recreational, educational, vocational, and social activities
  • Promote independence, self-advocacy, and active participation in the community
  • Provide transportation to community activities, events, and appointments
  • Complete required documentation, progress notes, and activity records
  • Manage outing budgets and activity-related expenses as needed
  • Ensure participant health, safety, and well-being, including providing basic first aid when necessary

Schedule
  • Full-Time

  • Flexible scheduling based on participant needs and program activities
  • Some evening and weekend availability may be required
Physical Requirements
  • Ability to lift up to 50 pounds
  • Ability to stand, walk, bend, and accompany individuals in community settings
  • Ability to safely transport participants using a personal vehicle
  • Ability to perform essential job functions with or without reasonable accommodation
