Job summary

Hope Services is seeking a dedicated staff member to provide group and individual support in the community, guided by each consumer's plan. You will help participants engage in activities, travel with clients, and model positive community involvement.

The role emphasizes advocacy, documentation, and collaboration with families and staff to ensure safety and learning opportunities for adults with developmental needs.

Qualifications

• High school diploma or GED required with related experience considered.
• Excellent communication and documentation skills including use of computer software.

Responsibilities

• Provide group and/or individual support in the community per the Individualized Personal Plan.
• Develop opportunities for consumer participation and model interactive relationships.
• Facilitate activities to build relationships, skills, and experience in the community.
• Transport clients and travel with them within the local community.
• Act as advocate and complete required forms and documentation.
