Job summary

Bronx Community Health Network is seeking a Community Health Worker (CHW) Supervisor to lead and evaluate CHWs, coordinate staffing, and ensure effective community-based service delivery addressing social determinants of health in the Bronx.

You will collaborate with clinical teams, oversee outreach, document encounters in the EHR, and report on program performance while ensuring HRSA/FQHC compliance and strong patient engagement.

Qualifications

  • Bachelor’s degree in health education, public health, community health, or related field.
  • 3+ years in community health, coordination, population health, case management, or social services.
  • 1–3 years of supervisory/leadership experience.
  • Experience with medically underserved populations and FQHCs or community settings.

Responsibilities

Description

The Community Health Worker (CHW) Supervisor is responsible for the supervision, coordination, and management of Community Health Workers (CHWs) who support patients and families by addressing social determinants of health, improving access to care, and promoting positive health outcomes. The CHW Supervisor provides leadership, coaching, training, and operational oversight to ensure effective delivery of community-based services aligned with the mission. The CHW Supervisor collaborates with clinical teams, care management staff, behavioral health providers, community organizations, and external partners to improve patient engagement, care coordination, health equity, and access to essential resources.

Bronx Community Health Network (BCHN) was founded in 1996 to ensure that low-income and medically underserved residents of the Bronx have access to high-quality and comprehensive healthcare services. BCHN is a not-for-profit, community-based organization and Federally Qualified Health Center. The organization provides a wide range of comprehensive, coordinated primary medical, oral, and mental health care and related diagnostic, pharmacy, social support, and enabling services for medically underserved/uninsured residents.

BCHN’s mission is to improve the health of people and families in the Bronx; provide access to affordable, quality health care, especially for uninsured people; promote disease prevention, early treatment and healthful lifestyles; and obtain financial and other resources for programs and services. BCHN’s service area includes the entire borough of the Bronx, including all 25 zip codes. Within this area, Health Center sites focus on the low-income and underserved populations.

OVERVIEW OF PRIMARY RESPONSIBILITIES
  • Supervise, coach, and evaluate Community Health Workers and related support staff.
  • Conduct regular staff meetings, performance evaluations, and individual supervision sessions.
  • Develop staffing schedules and assign caseloads to ensure coverage and efficiency.
  • Monitor productivity, performance metrics, and quality of services provided by CHWs.
  • Identify training and professional development opportunities for CHW staff.
  • Promote a culture of teamwork, accountability, and patient centered care.
  • Oversee outreach activities designed to connect patients to healthcare services and community resources.
  • Ensure CHWs assist patients in addressing barriers to care, including housing, food insecurity, transportation, employment, education, and other social needs.
  • Build and maintain effective relationships with community-based organizations, social service agencies, schools, faith-based groups, and public health entities.
  • Identify resources and referral opportunities for patients and families.
  • Represent the organization at community meetings, events, and collaborative initiatives.
  • Promote health education and community engagement activities.
  • Ensure timely and accurate documentation of patient encounters, referrals and outcomes in the electronic health record (EHR) and other systems.
  • Prepare reports related to program performance, outreach activities, and social determinants of health interventions.
  • Track and monitor referrals, resource utilization, and patient outcomes.
  • Ensure compliance with HRSA, FQHC, state, and federal regulations.
  • Maintain confidentiality and comply with HIPPA requirements.
  • Support data collection and reporting for Uniform Data System (UDS) and other grant funded programs.
  • Assist with audit preparation and performance reviews related to community health programs.
  • Maintain compliance with organizational and regulatory documentation standards.
Experience and Qualifications:
  • Bachelor’s degree in health education, Public Health, Community Health, Health Promotion, Nursing, Social Work, or a related field.
  • Minimum of three (3) years of experience in community health, coordination, population health, case management, or social services.
  • Minimum of one (1) to three (3) years of supervisory or leadership experience.
  • Experience working with medically underserved, vulnerable, or divers populations.
  • Experience in a Federally Qualified Health Center (FQHC), healthcare organization, or community-based setting.
Special Skills and Knowledge:
  • Knowledge of social determinants of health and community resource systems.
  • Understanding of population health and care coordination principles.
  • Strong leadership, coaching, and team building skills.
  • Excellent communication, interpersonal, and conflict resolution abilities.
  • Ability to build partnerships with community organizations and stakeholders.
  • Experience with electronic health records and data reporting systems.
  • Experience with value-based care, care management, and population health initiatives.

Salary Range:

The base salary range for this position is $66,000 – $70,000 annually.

The total compensation for the finalist selected for this role will be determined based on various factors, including but not limited to scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation. The base salary range listed above is a good faith determination of potential base compensation at the time of this job advertisement and may be modified in the future.
