Job summary

Baptist Health in Jacksonville is seeking a Community Health Worker to join its care coordination team at Downtown Jacksonville. This full-time, day-shift role focuses on improving patient outcomes and strengthening community health.

You will bridge between providers, community services, and patients, build trusted relationships, support continuity of care in a primary care medical home, and educate on chronic conditions to reduce disparities and improve quality of life for the communities
