Baptist Health in Jacksonville invites applications for a Community Health Worker to support care coordination and access to services across the care continuum. You will bridge providers, community organizations, and patients, fostering trusted relationships and helping patients navigate chronic disease management in a primary care home model.

Responsibilities include health education, addressing cultural and language needs, reducing disparities, and ensuring continuity of care for populations

Formación

• 1-2 Healthcare experience required.
• High School Diploma required.

Responsabilidades

• Support patients within a primary care medical home model, ensuring continuity of care.
• Provide health education and guidance for managing chronic conditions such as Diabetes, Cardiovascular disease, and COPD.
• Help reduce health disparities and improve overall quality of life for the populations served.
