This position involves working with STAR / CHIP members with Special Healthcare Needs. As a Health and Social Services Coordinator, you will be a vital part of an Integrated Care Model by relaying pertinent information about members’ needs, advocating for the best care, and ensuring they receive appropriate services.

Must be based in Texas and able to travel to the local office/community about 10% of the time based on business needs.

If you reside in Texas, you will have the flexibility to telecommute.

Primary Responsibilities :
  1. Assess, plan, and implement care strategies tailored to individual patients, aiming for the least restrictive appropriate level of care.
  2. Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  3. Manage the care plan throughout the continuum of care as a single point of contact.
  4. Communicate with all stakeholders to ensure coordinated, quality care and timely services for members.
  5. Advocate for patients and families to ensure their needs and choices are fully supported by the healthcare team.

Required Qualifications :
  • High School Diploma / GED or higher.
  • At least 3 years of experience in a healthcare role.
  • Intermediate proficiency in Microsoft Office (Word, Excel, Outlook).
  • Ability to secure Protected Health Information (PHI).
  • Access to a designated quiet workspace with high-speed internet.
  • Ability to travel within assigned regions up to 10% for member visits and team meetings.
Preferred Qualifications :
  • Certified Community Health Worker (CHW).
  • Experience with Texas STAR and CHIP programs.
  • Case management and call center experience.
  • Experience working with members with serious mental illness (SMI).
  • Bilingual in English and Spanish or Chinese/Mandarin.

The salary range for this role is $19.86 to $38.85 per hour, depending on full-time employment, experience, and other factors. Benefits include a comprehensive package, incentives, stock purchase options, and retirement contributions.
