Job summary

A community health organization in Lynn is seeking a Community Health Worker to support individuals with complex substance use disorders. Base pay range

$18.22/hr - $28.14/hr

Lynn Community Health Center is proud to work with the Massachusetts Bureau of Substance Addiction Services (BSAS) Reintegration Grant for Black and Latino Men. The Community health worker works directly with the Middleton House of Corrections, which includes three facilities. The Essex County Pre-Release and Re-Entry Center is a secure facility for men, including a substance use treatment unit. Data reports from the Sheriff's Department show that inmates released from the CASE Units consistently have the highest recidivism rates compared to inmates released from other substance use treatment units and all other units at all three facilities.

In This Role You Will
  • Go behind the walls to visit and conduct case management sessions with incarcerated patients.
  • Interact with enrollees in their homes, neighborhoods, and communities as necessary, to identify and resolve member’s barriers to receiving recommended health services, adhering to treatment recommendations made by providers, and becoming effective managers of their health.
  • As needed: schedule provider visits on behalf of enrollees, accompany enrollees to visits, ensure access to other community and government-based services.
  • Guide and teach caregivers about symptom response plans.
  • Act as an advocate and liaison between the enrollee/family and community service agencies (i.e. schools, Department Human Services, homeless shelters, hospitals, support groups, etc.).
  • Provide ongoing follow-up, basic motivational interviewing, and goal setting with enrollees/families.
The Successful Applicant Will Offer
  • Experience working in a clinical or special service setting with Medicare, Medicaid, or Special Needs populations.
  • Community Health Worker, Engagement Specialist, or Medical Assistant Certification.
  • Flexible work schedule – may need to work evenings/ occasional weekends to provide enrollee access and/or follow up.
  • Demonstrated success in working as part of a multi-disciplinary team including communicating and working with Physicians and Registered Nurses.
  • Fluency in a non-English language strongly preferred.
  • Strong work ethic built on a foundation of proactivity and teamwork.
