Job summary

Project FoodBox is hiring Community Health Workers to drive on-site member enrollments at community events across California. You will screen eligibility, explain how medically tailored meals help manage chronic conditions, and capture contact information for follow-up.

You'll work with outreach coordinators, engage diverse communities with empathy, and help connect members to resources while protecting privacy under CMS and HIPAA guidelines. Bilingual Spanish/English is a strong plus.

Qualifications

• Community Health Worker certification is required.
• Ability to handle sensitive health information with discretion and maintain confidentiality.
• Availability to work flexible hours, including evenings and weekends.
• Bilingual in Spanish and English is preferred.

Responsibilities

• Engage attendees proactively, screen for eligibility, and complete on-site enrollments
• Explain how medically tailored meals support chronic condition management, along with eligibility and next steps, in plain and accurate language
• Serve as a trusted liaison between Project FoodBox and the community, meeting people with empathy and reducing barriers to access
• Recognize other needs that surface in conversation and connect members to appropriate community resources
• Capture contact information from prospects who don't enroll on-site and hand qualified leads to outreach coordinators
• Complete enrollment forms and documentation accurately so no member is lost between the event and follow-up
• Support event setup, breakdown, and on-site logistics, and protect member privacy under CMS and HIPAA requirements throughout

We’re a Food as Medicine company working with health plans, community organizations, and local farms to deliver medically tailored groceries to people living with chronic conditions and communities facing food insecurity across California and New York. About the Role

We're hiring Community Health Workers to drive member enrollment at community events and serve as a trusted, culturally responsive presence in the neighborhoods we serve. You'll engage prospective members directly, explain how medically tailored meals help manage conditions like diabetes and high blood pressure, screen for eligibility, and complete enrollments on-site. You'll be measured on completed enrollments, the accuracy of what you submit, and the quality of leads you pass along for follow-up. You'll work alongside our outreach coordinators, who own the events and partnerships you'll be staffing.

What You Bring

Required

• Community Health Worker certification is required
• Comfort starting conversations with strangers and asking eligibility and enrollment questions confidently
• Ability to handle sensitive health information with discretion and maintain confidentiality
• Attention to detail when completing enrollment forms and documentation
• Availability to work flexible hours, including evenings and weekends, and to stand for extended periods with light lifting

Preferred

• Bilingual in Spanish and English
• Lived experience in or a strong connection to the communities we serve
• Experience as a Community Health Worker, promotor(a), or in enrollment, outreach, or event staffing
• Familiarity with Medicaid or health plan enrollment processes

The expected pay range for this role is $24 - $28 hourly, plus performance bonus and benefits. Compensation for this role will depend on various factors, including qualifications, skills, and location.
