Job summary

Mercy is seeking a Community Health Worker to serve as a trusted liaison between health and social services and the community. The role emphasizes outreach, client engagement, and documentation in an EMR system to improve access to services and cultural competence.

Ideal candidates bring 1–3 years CHW experience or equivalent, strong communication, and the ability to work independently in a Mercy-affiliated setting. Transportation is required to reach clients across our service area.

Qualifications

• High School Diploma or equivalent required.
• Mercy-provided Community Health Worker training certificate within 6-12 months.
• Basic Life Support certification preferred/required.

This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Certification/Registration:

Must be proficient with Microsoft Applications and be able to document in an EMR.
