Sorogi in Washington, DC is seeking Community Health Workers for both full-time and part-time openings. This in-person role pays $25 per hour with a monthly travel stipend to site and partner locations, and supports up to 45 participants per CHW.

As the main point of contact, you will recruit and enroll pregnant women, verify eligibility, help with weekly produce box pickups via the SorogiEAT app, collect data, and conduct home visits.

Qualifikationen

• High school diploma or GED.
• Experience in community outreach, health, or social services.
• Lives in or knows DC Wards 5, 7, and 8 (strongly preferred).
• Community Health Worker certification and a second language are a plus.

Aufgaben

• Recruit and enroll pregnant women through clinics, community partners, and events.
• Verify eligibility at enrollment and every six months (SNAP, WIC, Medicaid/CHIP, or food-insecurity screening).
• Help participants pick up and redeem weekly produce boxes in the SorogiEAT app, and follow up after missed pickups.
• Collect and report required data accurately and on time.
• Make home visits to postpartum and mobility-limited participants and help at pickup sites.
• Conduct weekly phone call check-ins and follow-up with participants.
• Connect participants to WIC, healthcare, and other community resources.
