The Community Health Worker II engages with community members, with an emphasis on underrepresented populations, to identify health and social risk factors and ensure access to appropriate resources and support services. This role, funded through the pediatric oncology Catalyst grant, is responsible for coordinating and facilitating communication between community members and healthcare teams to promote improved health outcomes and continuity of care and appropriate engagement in clinical trials. The position develops and provides culturally tailored education and guidance and conducts outreach and community engagement to build trust and connect individuals to services.
This is a grant-funded, term-limited position contingent upon the duration of funding. The role requires travel and the ability to work weekends and other non-traditional hours as needed. Spanish-speaking abilities preferred. Previous professional experience under the title/job function similar to a Community Health Worker is highly preferred.

Minimum 19.02/hour - Maximum 30.44/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
