• Associate’s/Technical Degree or equivalent combination of education/related experience (Preferred)
• Successful completion of a Community Health Worker formal training program from an accredited college or other educational institution (Preferred)

Responsibilities

• Assist in offering interpretation and translation services, provide culturally appropriate health education and information, and offer informal counseling and guidance on health behaviors, establishing trusting relationships and conducting intake interviews, enrolling or referring patients into local services.
• Follow up with patients via phone calls, make home visits, assist patients with completing applications and registration forms, conduct eligibility determination, and help patients set goals and attend appointments.
• Help patients connect with transportation resources, exhibit excellent working relations, and communicate the organization’s mission.
• Work closely with medical providers to ensure coordinated care, follow patients from initial identification through closure, and follow established procedures.
• Provide consistent communication to management, attend staff meetings, trainings, and other meetings as requested.
• Perform other job‑related duties as assigned.
