Position Summary
  • Community Health Care Workers (CHW) serve as a bridge between the community members and the healthcare system through outreach and education, helping members attain better health outcomes and improve relationships with providers. This position works under the guidance of a designated professional.
  • Candidates must reside in Marion or Wayne Clay, Richland, Lawrence, Edwards, or Wabash County, IL.
  • Schedule: 8:00 am - 4:30 pm CST.
Primary Responsibilities
  • Conduct home visits with members with chronic conditions to reinforce care plans, considering cultural, linguistic, and religious factors, and to reduce unnecessary hospitalizations.
  • Educate healthcare professionals on social determinants impacting health and treatment plans via various communication channels.
  • Build positive relationships with members, providers, and community partners, working to reduce cultural and socio-economic barriers.
  • Assist with care plan implementation, member education, and develop strategies to address health and social needs.
  • Help members identify socio-economic issues affecting their health and develop management plans and goals.
  • Identify community system gaps and assist members in utilizing community services, including scheduling and application assistance.
  • Support medication adherence by reviewing medication lists and assisting with refills.
  • Teach disease self-management, including nutrition and symptom tracking.
  • Accompany members to appointments as needed.
  • Maintain accurate records in electronic health systems, compile reports, and complete administrative tasks.
  • Travel extensively to member homes, community locations, and outreach sites.
Required Qualifications
  • 3-5 years of community-related experience.
  • Reside in Marion or Wayne County, IL.
  • Reliable transportation and willingness to travel up to 70% within these counties; mileage reimbursed.
Preferred Qualifications
  • 1-2 years of clinical service experience, preferably in community/outpatient settings.
  • Community Health Worker certification or equivalent training.
  • Bilingual abilities.
  • Experience serving Medicaid populations in urban or rural settings with knowledge of local resources.
  • Additional training in mental health, trauma-informed care, HIPAA, motivational interviewing, and medical terminology.
  • Compliance with local requirements such as first aid, CPR, TB screening.
  • Proficiency in computer use, including Excel and Electronic Health Records.
Education
  • High School Diploma or GED required.
Work Hours and Compensation

Full-time, 40 hours per week. Pay range: $18.50 - $38.82 per hour, depending on experience and other factors. Eligible for bonuses and incentive programs.
