Job Overview:

The Community Health Worker will primarily maintain a presence in the community with patients to promote, motivate, and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers and meet their health goals.

Qualifications:

Required:

  • High School Diploma or equivalent

Scheduling Requirements:

  • Full-Time, Evening - 3P-11P, subject to change based on business need.
  • Working Location: Long Branch, NJ

Essential Functions:

  • Meet patients in clinics, hospitals, ERs, and other locations to conduct needs assessments using Social Determinants of Health tools.
  • Schedule weekly follow-up calls and home visits.
  • Assist patients with social issues such as homelessness, substance abuse, hunger, transportation, and other social determinants impacting their lives.
  • Help patients organize records and make follow-up appointments with healthcare providers.
  • Support patients in accessing medical care and achieving health goals, including planning healthy meals and exercise routines.
  • Assist patients in accessing resources like quality care, childcare, food, housing, and transportation.
  • Maintain relationships with community-based organizations.
  • Travel to meet members at various locations, including churches, shelters, and residences.
