Job description

Community Health Worker - Endocrinology, Newton

What You'll Do:

  1. Empower Patients: Help individuals with diabetes access necessary resources such as medication assistance, insurance support, and follow-up care.
  2. Reduce Diabetes Burnout: Offer guidance and encouragement to help patients stay engaged in their care.
  3. Strengthen Community Connections: Connect patients with senior centers, local support groups, financial assistance programs, and food security resources.
  4. Collaborate with Healthcare Teams: Work alongside the endocrinology and iCMP care teams to develop a seamless bridge of care for patients.

Qualifications:

  • High School Diploma/GED required; Associate or Bachelor's degree preferred.
  • Prior experience in healthcare, social work, or a related field preferred.
  • Valid driver's license and ability to travel within the community as needed.

What We're Looking For:

  • Experience in community outreach, patient advocacy, or health coaching preferred.
  • Knowledge of community resources and social services that support individuals with diabetes.
  • Bilingual skills (Spanish or Haitian Creole preferred) to better serve our diverse patient population.
  • Excellent communication, problem-solving, and organizational skills to manage a caseload of patients effectively.
