Salary Range: $20.96 - $29.60 Hourly

Job Overview:

The Community Health Worker will maintain a presence in the community to promote, motivate, and improve healthy living. They will assist patients in utilizing resources and accessing services to overcome barriers and meet health goals.

Scheduling Requirements:

  • Full-Time, Day - 9A-5P, subject to change based on business needs.
  • Working Location: Monmouth Medical Center, Long Branch, NJ

Essential Functions:

  • Conduct needs assessments with patients in various settings using Social Determinants of Health Tools.
  • Schedule follow-up calls and home visits.
  • Assist with social issues such as homelessness, substance abuse, hunger, and transportation.
  • Help organize patient records and appointments.
  • Support patients in accessing healthcare and achieving health goals through planning and resource connection.
  • Maintain relationships with community-based organizations.
  • Travel to meet members at various locations, including churches, shelters, and residences.
