Job Overview:

The Community Health Worker will primarily maintain a presence in the community with patients to promote, motivate, and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

Qualifications:

Required:

• High School Diploma or equivalent

Preferred:

• Bi-lingual in Spanish is strongly preferred

Scheduling Requirements:

• Full-Time, Day shift
• Working Location: CBMC- Livingston, NJ

Essential Functions:

• Meet patients in clinics, hospitals, ERs, and community settings to conduct needs assessments using Social Determinants of Health tools.
• Schedule follow-up calls and home visits.
• Assist patients with social issues such as homelessness, substance abuse, hunger, transportation, and other barriers.
• Help organize patient records and coordinate follow-up appointments.
• Support patients in achieving health goals through planning meals and encouraging exercise.
• Connect patients with resources like healthcare, childcare, food, housing, and transportation.
• Maintain relationships with community-based organizations.
• Travel to meet members at various locations, including churches, shelters, and residences.
