Community Health Worker (CHW) Care Manager

Join to apply for the Community Health Worker (CHW) Care Manager role at Greater Lawrence Family Health Center.

GLFHC is seeking a CHW Care Manager to provide onsite and community harm reduction and screening services, including outreach, prevention, counseling, and linkage for groups at high risk for HIV/AIDS, STIs, Viral Hepatitis, and latent tuberculosis, as defined by the Massachusetts Department of Public Health. The role involves phlebotomy training within the first 18 months, screening for HIV, Hepatitis C, Syphilis, Chlamydia, Gonorrhea, and TB, and linking positive cases to care. The CHW will also provide overdose education, Narcan enrollment, and syringe services.

Job Responsibilities Include:
  1. HIV/STI/HCV Outreach and Screening: Conduct outreach, assess risk factors, provide counseling, facilitate interventions, and educate on harm reduction materials.
  2. Short Term Navigation: Link clients to medical and behavioral health services, assist with barriers, and coordinate appointments and support services.
  3. Overdose Education and Narcan Enrollment: Provide overdose prevention education, conduct trainings, and enroll clients in Narcan programs.
  4. Syringe Services (City of Lawrence only): Engage PWID, distribute sterile equipment, educate on safe use, and dispose of used equipment properly.
  5. Administrative Responsibilities: Support community events, ensure data accuracy, participate in quality activities, and maintain documentation and resources.
Qualifications:
  • Certified phlebotomist or willing to train within 18 months.
  • Certified Application Counselor or willing to train within 6 months.
  • Valid Massachusetts Driver’s license and reliable transportation.
  • Bilingual/bicultural (English/Spanish) preferred.
  • Strong initiative, problem-solving, communication, and computer skills.
  • Ability to work evenings and weekends.
Education & Experience:
  • Associates Degree in Human Services or two years relevant experience.
  • Experience with priority populations and knowledge of substance use, HIV, STI’s, HCV, and community resources.
