JOB DESCRIPTION

POSITION TITLE: Community Health Worker

DEPARTMENT: Case Management

FLSA STATUS: Non-exempt

REPORTING RELATIONSHIP: Director, Case Management

Job Summary: The Community Health Worker (CHW) supports individuals and families in accessing healthcare and community resources. CHW builds trusting relationships with community members, provides health education, assists with care coordination, and helps address barriers such as transportation, housing, food access, and healthcare services.

Essential Functions:

• Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
• Connects clients with healthcare, social services, and community resources.
• Provide health education and information in an understandable and culturally appropriate manner.
• Conducts outreach to individuals and families in the community.
• Helps clients navigate healthcare systems, appointments, referrals, and benefits.
• Identify social and environmental barriers that may affect health and well-being.
• Assists clients with applications for healthcare, food, housing, transportation, and other assistance programs.
• Follows up with clients to assess needs and encourage participation in recommended services.
• Maintains accurate and confidential client records.
• Communicates and collaborates with healthcare providers, social service agencies, and community organizations.
• Participates in community events, health screenings, educational programs, and outreach activities.
• Advocates for clients and help reduce barriers to receiving appropriate care.

Minimum Qualifications: 18 years of age.

Knowledge, Skills & Abilities: Preferred CHW certification completed or in progress. Ad ditional education in healthcare, human services, social work, or a related field is preferred. Experience in a physician health care office or similar setting, preferred. Clear oral expression: oral comprehension includes listening to and understanding others; speaking clearly such that others understand; deductive and inductive reasoning skills; ability to write clearly; use of technology such as EMR, Athena and Cerner preferred. Ability to work with diverse patient and staff populations with sensitivity to cultural and spiritual influences impacting patient care. Efficiently manage time with minimal supervision.

Working Conditions: Work in an unpredictable environment encountering stressful situations and exposure to body fluids and infectious diseases.

Physical Requirements: Constantlysee/visual acuity, handle/grasp/feel, talk/hear, taste/smell. Frequently sit. Occasionally lift/carry 1 to 25 lbs.
