Akido in California is seeking a Community Health Worker to support members with complex medical and social needs, helping them manage health and wellbeing through education, peer support, and shared experiences.
The CHW will manage a caseload, build rapport, coach patients, accompany them to appointments, connect to programs, document encounters, and enroll eligible members into the Akido ECM Program. Travel 50–75% is required.

The Community Health Worker (CHW) will support members with complex medical and social needs with managing their own health and wellbeing. CHW will assist in motivating behavioral changes in patients to improve health outcomes in members through education, peer support, and the relaying of shared experiences.
Compensation: $25/hour
Hourly pay range
$22 — $25 USD
