The Opportunity

Join our Street Medicine team as the vital link between street‑based outreach and the healthcare system. This role splits between office‑based administrative work (70%) and field clinical support (30%), focusing on care coordination and navigation for our most vulnerable patients through our Enhanced Care Management (ECM) program.

What you’ll do

Administrative & Care Coordination (70%)

• Manage referrals to specialists: initiate, schedule, track, and ensure follow‑up
• Secure prior authorizations for medications, imaging, and specialist visits
• Verify insurance eligibility (Medi‑Cal, Medicare) and assist with enrollment issues
• Maintain medical supply inventory for field outreach
• Document patient encounters, referrals, and authorizations in EHR system
• Support ECM program through patient tracking, scheduling, and care coordination
• Serve as office point of contact for patients, pharmacies, and community partners
• Other duties as assigned

Field Clinical Support (30%)

• Accompany provider (MD/DO/NP/PA) on street outreach to encampments, parks, and shelters
• Build trusting relationships with patients and perform field intake
• Take vital signs, assist with clinical procedures, wound care, and specimen collection
• Prepare and administer medications/vaccinations under supervision
• Other duties as assigned

Who you are

Required

• Medical Assistant certification from accredited program
• Current BLS certification
• High School Diploma or equivalent
• Valid California Driver’s License with clean record
• Strong computer and organizational skills
• Exceptional empathy, compassion, and communication abilities

Preferred

• Bilingual (English/Spanish)
• 1+ years clinical administrative experience with prior authorizations/referrals
• Additional certifications (EMT, CNA, Phlebotomy)

Salary range: $58,240 USD - $58,240 USD
