Community Health Worker – Bilingual Spanish/English
Location: Remote with local travel – Adams, Lancaster, Lebanon or York County, PA
Travel: 50–75% local travel; reimbursed
Schedule: Monday–Friday, regular business hours with occasional evenings/weekends for community events
Position Overview
We are seeking a compassionate, community-focused Community Health Worker (CHW) to help members overcome barriers to healthcare and connect with the services and resources they need. This role combines member outreach, education, care coordination and community engagement, with a focus on preventive health, maternity, care transitions, chronic conditions and health-related social needs.
The ideal candidate is bilingual in Spanish and English, has strong ties to the community served, and is passionate about working with Medicaid and at-risk populations.
Key Responsibilities
- Conduct face-to-face and telephone outreach to members regarding preventive health, care gaps, maternity, care transitions, chronic conditions and health-related social needs.
- Educate members on preventive healthcare services and help them better understand and navigate available benefits and resources.
- Conduct Social Determinants of Health (SDOH) assessments and identify barriers that may impact a member's ability to access care.
- Assist members with scheduling healthcare appointments and connecting to appropriate community resources and services.
- Coordinate with healthcare providers, community agencies and social service organizations to help members access needed care and support.
- Build trusted relationships with members and empower them to take an active role in their health and wellness.
- Partner with organizations such as WIC, Head Start programs, community centers, homeless shelters and other community-based organizations.
- Identify members who may benefit from additional care management services and make appropriate referrals.
- Participate in community outreach initiatives and represent the organization at local events.
- Assist in developing educational materials and outreach programs designed to effectively reach at-risk populations.
- Document outreach activities, outcomes, referrals and member interactions.
- Provide accurate information regarding available healthcare benefits and services.
Required Qualifications
- High school diploma or GED.
- Minimum 1 year of Community Health Worker experience or related experience.
- Bilingual in Spanish and English – required.
- Lived experience or strong familiarity with the community being served.
- Must reside in Adams, Lancaster, Lebanon or York County, Pennsylvania.
- Ability to travel locally 50–75% of the time.
- Must obtain Certified Community Health Worker (CCHW) certification through the Pennsylvania Certification Board within 12 months of starting.
Preferred Qualifications
- Experience working with Medicaid populations or Medicaid regulations.
- Healthcare customer service or member services experience.
- Experience supporting at-risk or underserved populations.
- Familiarity with community resources and social service organizations.
