Arkansas Children's is seeking a Community Health Worker for the Primary Care Clinic, to coordinate care for families between ACH providers and community resources. The role requires a Bachelor's degree or four years related experience, and will involve working between the Springdale campus and Rogers Harvey Clinic.
The CHW will help families navigate health and social services, provide coaching and referrals, and support access to medications and benefits.

The Community Health Worker (CHW) provides coordination of care for patients and families between ACH primary care providers and appropriate community resources. The CHW will help families navigate various systems of care while connecting them to the medical, social, and mental health services for which they may be eligible. CHWs serve as a liaison between the family, providers, and community based organizations (CBO). They should have a strong understanding of the healthcare delivery system and help patients overcome access challenges. The employee will provide coaching, support, and guidance to the patient as they work towards their health care goals.
Bachelor's degree from four-year college or university; Or four years related experience and/or training; or equivalent combination of education and experience.
