Saint Francis Healthcare System is seeking a Full Time Outreach and Care Coordination professional to engage patients in preventive and ongoing healthcare. You will assist in addressing clinical and non-clinical needs, connect to community resources, and support treatment adherence and self-management.

Coordinates outreach, education, and care coordination services to engage patients in preventive and ongoing healthcare, support closure of care gaps, improve clinical outcomes, and promote effective use of healthcare resources. Assists patients in addressing clinical and non-clinical needs by facilitating access to community resources, supporting treatment adherence and self-management, and helping remove barriers to achieving care plan goals.
Type: Full Time (80 hours per 2 week pay period, with benefits)
