Assists patients in addressing clinical and non-clinical needs by facilitating access to community resources, supporting treatment adherence and self-management, and helping remove barriers to achieving care plan goals.

Type: Full Time (80 hours per 2 week pay period, with benefits)

Typical hours for this position: Monday-Friday 8:00am-4:30pm

Education

• High School diploma or GED- required

License/Certification

• BLS- required
• Community Health Worker certification- preferred

Experience
