Saint Francis Healthcare System is seeking a Community Health Worker (CHW) / care coordinator to engage patients in preventive and ongoing care, close care gaps, and connect them with community resources to improve outcomes. Typical hours are full-time, Monday–Friday 8:00am–4:30pm. Requires a high school diploma or GED and BLS certification; CHW certification is preferred.
