Mowana Care is hiring compassionate, relationship‑driven Community Health Workers (CHWs) to support pregnant and postpartum individuals and families through outreach, care navigation, maternal wellness support, and community‑based advocacy. We are seeking individuals who can build trust with families, healthcare providers, and community organizations while helping underserved communities navigate healthcare and social support systems.

Full‑Time and Part‑Time Opportunities Available

Schedule
  • Flexible scheduling within standard weekday business hours
  • Occasional evenings/community events may be required
  • Hybrid and remote opportunities available. Candidates do not need to reside in Hunterdon or Mercer County; however, hybrid staff must be willing to travel occasionally throughout rural service areas for outreach, meetings, home visits, community engagement, and partner coordination.
Key Responsibilities
  • Conduct community outreach and engagement
  • Build trust‑based relationships with clients and families
  • Conduct Social Determinants of Health (SDOH) screenings
  • Provide care navigation and referrals
  • Support maternal health and behavioral health coordination
  • Conduct follow‑up support and resource connection
  • Collaborate with healthcare providers and community organizations
  • Maintain accurate documentation and client records
  • Participate in community events and outreach activities

Required Training

All hired Community Health Workers are required to complete onboarding requirements and NJDOH‑required Community Health Worker training, including enrollment in and/or completion of the Colette Lamothe‑Galette Community Health Worker Institute (CLG‑CHWI) training program during the grant period.
