Job summary

Raising Hope Inc in Providence, Rhode Island is seeking a Community Health Worker to link individuals with disabilities to vital community resources, including medical care and social services.

The ideal candidate will have a strong ability to communicate in English, preferably also in Spanish, and must be familiar with Rhode Island neighborhoods, dedicated to outreach and support for Medicaid recipients.

We are seeking a Community Health Worker to connect persons with disabilities to community resources, including medical care, social services, finance, insurance, and other health related needs. Ideal candidate must reside or familiar with Rhode Island neighborhoods. This position requires outreach to different communities, and direct contacts with Medicaid recipients.

Qualifications

• Certified Community Health Worker by RICB
• Ability to communicate in English, and other languages, preferably Spanish
• Current CNA or Home Care experience a plus
• Ability to prioritize and multitask
• Excellent written and verbal communication skills
