What You'll Do

Licenses/Certifications
  • Community Health Worker (CHW) certification through a college-level program.
State-Specific Requirements
  • Ohio Plan: CHW certification through a college-level program or LPN or LCSW license required. Valid driver’s license required.
  • Michigan Plan: CHW certification preferred but not required. Valid driver’s license required.
  • Illinois Plan: CHW certification preferred but not required. Valid driver’s license required.
  • Medicare: CHW certification through a college-level program or LPN license required.
  • Valid driver’s license required.
  • Kentucky Plan: CHW certification preferred but not required. Valid driver’s license required.
Environmental Job Requirements and Working Conditions
  • Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 19500 HWY 249, Suite 570 Houston, TX 77070.
  • The total compensation target pay range for this role is: $23 - $28 per hour. The range represents our national target range for this role.
