Job description

Job Title: Clinical - Care Navigator

Duration: 3 months Contract (Potential to Extend)

Location: Remote – SC (Greenville, Myrtle Beach, and the Lowcountry are preferred, bilingual candidates are a plus. Columbia area is a last resort, willing to travel required.)

Shift: 8:00am - 5:00pm EST, Mon-Fri

Pay Rate: $29/hr

Job Duties:

1. Schedule 8 hours daily.
2. Make calls, visit, and coordinate care for members.
3. Work driven by state metrics, all time-sensitive.
4. Communicate via Teams, calls, or texts.
5. Work with "buddies" for co-visits.

Required Skills/Experience:

1. Ability to adapt to changing policies and procedures.
2. Work independently to meet metrics.

Education:

Bachelor’s Degree and Certified Community Health Worker (CHW)

Must-Haves:

Bachelor's degree with CHW certification

Nice-to-Haves:

Background in Behavioral Health (BH) is helpful.

Willing to travel; no blind visits.
