Under the supervision of the Community Outreach Coordinator, and the direction of the Director of Community Relations, the Community Health Worker (CHW) provides essential support for hospital and grant-funded health initiatives, focusing on improving healthcare access and outcomes for everyone in the Hunterdon Health service area with a particular focus on uninsured, and underinsured populations. The CHW serves as a bridge between the clinical setting and the community, engaging patients directly to address social determinants of health and ensure connection to preventive services, screenings, medications, and community resources.
