Hiring Range: $38,000 - $45,000

Position Summary

The FIT Program Community Health Worker will provide outreach, counseling, navigational, and case management services to individuals recently released from incarceration with chronic medical, psychiatric and/or substance use conditions who present at or are recruited by the FIT Program. This position is based at the UNC REACH (Recovery and Access to Comprehensive Health) Enhanced Primary Care clinic and Oak City Cares in Raleigh.

Minimum Education and Experience Requirements

Demonstrated possession of the competencies necessary to perform the work.

Management Preferences
  • Experience working with health care systems, mental health and substance use treatment programs, community-based organizations.
  • Experience working as part of a multi-disciplinary team, collaboratively practicing with therapists, psychiatrists, PCPs, health workers, health educators, and/or other health professionals.
  • Excellent organization, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators.
