Medical Search Associates presents an opportunity for a Social Worker BSW to join a health system in east central Ohio, with a Monday–Friday day schedule and home visits in surrounding counties.

The role emphasizes discharge planning, patient advocacy, and collaboration with clinicians to improve health outcomes. A Master's in Social Work or Counseling and an Ohio license are required.

Qualifications

• Master's degree in Social Work or Counseling required.
• Ohio license required.
• 1–2 years minimum experience as licensed social worker.
• Valid driver's license and proof of auto liability insurance.

Responsibilities

• Provides professional social work services through assessment and resolution of problems impacting health and well-being, including discharge planning.
• Utilizes supportive counseling techniques and crisis interventions; facilitates groups as needed.
• Participates in community boards/tasks forces when appropriate.
• Keeps abreast of changes in laws affecting social work practice.
• Completes psycho-social assessments using a systems approach.
• Facilitates completion of Advance Directive forms and related pre-admission screenings.
• Makes referrals to Medicaid waiver programs as appropriate.
• Observes signs of abuse/neglect and follows state hospital policies.
• Advocates for patient rights and involves patient/family in decisions about care.
• Documents accurately within required timeframes and assists with staff onboarding.
• Collaborates with medical staff to ensure efficient care delivery.
