Position Summary

We are seeking a mission-driven, strategic, and relationship-oriented Community Health Sales Specialist to join our team. This individual will be instrumental in expanding access to our End-of-Life Care Navigation and Enhanced Care Management services by building and maintaining formal referral partnerships across healthcare and social service ecosystems.

This is a dual-impact role that combines business development, partnership management, and data-driven outreach. The ideal candidate has a deep understanding of local and regional healthcare market dynamics, high-risk and Medicaid populations, and a strong track record of fostering durable external relationships that convert into service referrals and patient engagement.

Key Responsibilities

Referral Network Development

  • Identify, engage, and formalize partnerships with organizations that serve potential referral populations, including hospitals, primary care practices, community-based organizations, behavioral health providers, managed care plans, and others.

  • Leverage market intelligence and patient population data to strategically target high-potential partners across both verticals (end-of-life care navigation and enhanced care management).

  • Collaborate with internal stakeholders to align outreach strategies with organizational priorities and geographic focus areas.

Ongoing Relationship & Partnership Management

  • Act as the primary point of contact for external referral partners, ensuring clear and consistent communication with internal clinical and operations teams.

  • Facilitate seamless referral workflows, service onboarding, and partner education.

  • Monitor partnership performance and satisfaction, troubleshoot issues, and identify opportunities for enhanced collaboration and service improvements.

Data Partnerships & Early Identification

  • Build and manage relationships with external data partners (e.g., health plans, hospital systems, HIEs) to enable timely identification and outreach to eligible patients.

Qualifications

  • Minimum of two (2) years in sales and marketing, preferably in healthcare

  • Proven track record in healthcare business development, community engagement, or partnership sales (preferably in Medicaid or complex care delivery settings).

  • Deep understanding of regional healthcare ecosystems and social determinants of health.

  • Exceptional relationship-building skills and the ability to work cross-functionally across clinical, operational, and external stakeholder teams.

  • Strong analytical skills; able to interpret population health data and apply insights to outreach strategy.

  • Knowledge regarding Value Based Care, SaaS products, and current AI offerings and capabilities

  • Excellent verbal and written communication skills.

  • Passion for advancing health equity and improving care for underserved populations.
