Job summary

Qualifications

• Illinois RN licensure is required.
• BLS for Healthcare Provider CPR certification required.
• 1–2 years in community health nursing or ambulatory care preferred.

Responsibilities

• Collaborates with providers in the delivery of patient care.
• Performs triage assessment for walk-in patients and calls.
• Delivers health education related to medications, diet, and prevention.
• Administers immunizations, contraception refills, and medications as ordered.
• Performs other duties as assigned.
