Erie Family Health Centers in Chicago is seeking a part-time Community Health RN to provide essential clinical support, nursing assessment, and patient care activities in our community-focused practice.

The role includes collaborating with providers, triage for walk-ins and calls, health education, and immunizations, with a schedule aligned to Erie’s hours. Bilingual English/Spanish is required to serve our diverse patient population.

Qualifications

• Current BLS for Healthcare Provider CPR certification is required.
• One year of experience in community health nursing or ambulatory care is preferred.
• Bilingual English/Spanish is required.

Responsabilités

• Collaborates with providers in the delivery of patient care.
• Performs triage assessment process of walk-in patients and telephone calls.
• Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
• Administers immunizations, applicable contraception refills, and medications as ordered by provider.
• Performs other duties as assigned.
• The work schedule for this role is aligned with Erie's hours of operations.

Erie Family Health Centers is seeking a part time (20 hours per week) Community Health RN to provide essential clinical support to patient care activities utilizing nursing education, assessment, and procedural skills.

Compensation is based on each candidate’s experience, skills and education within the range identified for the role.
