Job summary

InterCommunity, Inc. in East Hartford, CT seeks a Registered Nurse (RN) to join its primary care and psychiatry teams. You will provide direct patient care under the Clinical Practice Manager, perform clinical assessments, vital signs, and care planning for a diverse patient panel.

Required CT RN licensure, 1 year in public health, and 2 years in community health/addiction/psychiatric nursing. BLS/CPR certification; EPIC experience preferred. Bilingual Spanish encouraged but not required.

Qualifications

• Graduation from an accredited nursing program.
• Current Connecticut RN licensure.
• 1 year medical experience in a public health setting.
• 2 years of community health, addiction medicine and/or psychiatric nursing.
• BLS/ CPR certification.
• EPIC experience preferred.

Responsibilities

• Provides direct patient care as ordered by medical provider, including histories, vital signs, lab testing, phlebotomy, EKGs, wound care, and medication administration.
• Conducts nursing visits or educational groups as ordered by the medical provider.
• Maintains accurate, detailed reports and records; reconciles patients' medical information.
• Monitors and reports symptoms or changes in patients' conditions; assesses medication efficacy.
• Ensures gaps in care for screenings and chronic care tests are completed on schedule; coordinates with team to optimize outcomes.
