Main Purpose of Job

This position will implement maternal/child health grants at the local health department (LHD) level, provide case management services for pregnancy and postpartum community residents, assist with tracking grant-associated performance measures, and identify areas for program growth within the LHD's capacity. It will also include other nursing duties such as emergency preparedness response and vaccination clinics as needed. Additionally, the role involves providing direct nursing care in the Mobile Integrated Health Program by accompanying Paramedics to client homes to perform health and environmental assessments, monitor clients as directed by doctor’s orders, link clients to community resources, maintain accurate documentation and statistical data, and communicate with the director, paramedics, families, medical providers, and relevant agencies.
