It plays a key role in linking the Department of Health with community partners and guiding public health actions. As a Community Health Nurse, you will perform the following duties:

• Assess Community Needs:Evaluate county level assessments to identify public health trends and support data driven activities
• Oversee Initiatives:Develop initiatives using community data, public health protocols, and nursing guidelines to address health priorities
• Provide Education:Deliver health education programs on various public health topics to inform and guide the public
• Support Case Management:Offer outreach and case management services that promote patient health and safety
• Conduct Investigations:Investigate reportable diseases and outbreaks to prevent and control the spread of illness
• Coordinate Partnerships:Work with community organizations to implement disease prevention and control measures

Work Schedule and Additional Information:

• Full‑time employment, 37.5 hours per week
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
• Irregular Hours: This work infrequently requires statewide travel and irregular or flexible work hours including overnights, weekends, and holidays in accordance with policy.
• Telework: You will not have the option to telework in this position.
• Salary: Selected candidates who are new to employment with the Commonwealth of Pennsylvania will begin employment at the starting annual salary of $75,462.00 (before taxes).

QUALIFICATIONS

Special Requirements:

• You must possess active authorization to practice as a Registered Nurse in Pennsylvania.
• All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

Other Requirements:

• You must be able to perform essential job functions.

Legal Requirements:

• You must pass a background investigation.
• This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
