Qualifications

• High school diploma or equivalent required.

• 2+ years of volunteering or work in community/healthcare settings preferred.

Responsibilities

• Meets leadership and competency expectations for community health work.
• Conducts social needs screenings and links individuals to resources.
• Assist individuals in creating action plans and goal setting.
• Provide health, wellness, disease-prevention education.
• Mentor and coach AmeriCorps team members and support program implementation.
• Support daily program planning and deliver updates to the team.
• Review activities and ensure quality and adherence to standards.
• Consolidate program data and report performance metrics to leaders.
