Qualifications

• An active and valid Community Health Worker Certification.
• At least 1 year of experience in community health, behavioral health, peer support, or related roles.
• CPR/First Aid (Or willingness to obtain).

Responsibilities

• Develop comprehensive care plans with providers and agencies.
• Assist patients in navigating the healthcare system.
• Conduct home visits or outreach activities.
• Provide transportation for members to appointments.
