What You’ll Do

The Community Health Navigator, Emergency Department Follow-Up provides support in assisting patients with post-hospital care via telephonic and in-person support. You will:

  • Proactively engage patients post emergency department visits and provide education on ED avoidance and resources.
  • Conduct virtual and telephonic visits to support patients with in-person/community needs related to care coordination/discharge support (e.g., pharmacy fills, PCP coordination, SDOH needs, home visits).
  • Identify symptom worsening or concerns during post-discharge visits and escalate to the clinical team as appropriate.
  • Provide education on Imagine Pediatric Services.
  • Ensure patients have appointments for any needed follow-up.
  • Schedule callbacks for patients with Imagine clinical teams as appropriate.
  • Work in a multiplatform virtual environment as part of an interdisciplinary team.
  • Coordinate with facility discharge planners and Imagine Pediatrics’ Transition of Care Nurses to remove barriers to after-hospital care.
  • Attend daily huddles with your team to communicate patient needs and progress.
  • Liaise with hospital and health system staff to raise awareness of Imagine Pediatric programs and support.
  • Provide appropriate internal, social service, and community-based referrals.
  • Perform other duties as assigned.

What You Bring & How You Qualify

We are looking for candidates who are passionate about reimagining pediatric health care and ensuring every child with special health care needs receives the care and support they deserve. Requirements include:

  • Associates degree or higher
  • 2+ years of relevant clinical work experience
    • Emergency or inpatient settings preferred
    • Experience with medically complex patient populations preferred
    • Pediatrics preferred
  • One of the following designations: Certified Community Health Worker, Certified Medical Assistant, Home Health Aide, Licensed Practical Nurse
  • Knowledge of Medicaid population preferred

The hourly rate for this position ranges from $23 - 28 per hour, plus competitive benefits and eligibility to participate in an employee equity purchase program (as applicable). Compensation is determined based on job-related knowledge, skills, and experience.
