Responsibilities

• Care Coordination - participates as a member of the multi-disciplinary team to coordinate care and assist with plan of care progression.
• Scheduling - accurately schedules appointments and answers questions about preparations and directions.
• Documentation - completes required documentation linked to the plan of care accurately and timely.
• Communication - provides updates to member/family and links information to the plan of care.
• Consults with care team to address complex issues.
• Population Outreach - supports outreach to identify at-risk populations and close gaps.
