Penn Medicine Princeton Health (PMPMH) is seeking a full-time Community Health Navigator to work Monday through Friday within the Population Health department. The individual in this role will divide their time by working in the hospital setting as well as traveling throughout the community to serve our patients by focusing on rehabilitation plans that will assist in avoiding readmission.
Monday – Friday: 8am - 5pm (approximate)
Credentials: LCSW or LSW Required
Education: Master’s Degree Required
