Job summary

Missouri Highlands Health Care is seeking a Community Health Worker (non-exempt) to support patients in the community, working with medical providers and care teams to improve outcomes.

You will assist with Medicaid applications, help develop health management plans, and coordinate services across programs. This role emphasizes culturally competent outreach, documentation, and collaboration with staff to remove barriers to care.

Qualifications

• High school diploma or GED preferred.
• BLS – Basic Life Support certification is required or preferred.
• At least 3 years health or social services experience preferred.
• Valid driver's license and reliable transportation required.
• Background check and fingerprinting required.

Responsibilities

• Convey the purposes and services of a program to the user population and the impact.
• Help patients develop health management plans and goals.
• Assist patients with Medicaid applications and submissions.
• Follow-up with health management/care plans with patients and providers.
• Coach patients in managing chronic health conditions and self-care.
• Assist patients in understanding care plans and instructions.
• Document activities, service plans, and results per policies and procedures.
• Work collaboratively and effectively within a team.
• Establish positive, supportive relationships with participants and provide feedback.
• Help clients access health-related services and schedule appointments.
