MU Health Care is seeking a compassionate Community Health Navigator to help patients overcome barriers and connect them with resources to achieve health goals. The role emphasizes building strong relationships, coordinating transportation, and assisting with Medicaid eligibility while addressing social factors impacting health.

This onsite position is part of Care Management, with a focus on empowering patients and strengthening community connections to improve health outcomes in mid-Missouri.

Formación

• High school diploma or equivalent.
• CHW certification within one year as a condition of continued employment.
• Two years of healthcare experience.

Responsabilidades

• Identify and maintain an up-to-date directory of resources within patients' local communities to address social determinants of health and connect patients with appropriate services.
• Identify transportation resources and assist with coordination to optimize appointment attendance.
• Assist patients with determining eligibility for Medicaid and submission of required documentation.
• Identify and solve obstacles for adherence to treatment plans or patient-directed care goals.
• Engage with care teams regularly to understand patient needs and assist with non-clinical needs.
• May complete unit/department-specific duties as outlined in department documents.

Compensation:

Base Pay Range: $22.00 - $34.74 per hour, based on experience

DETAILED JOB DESCRIPTION

• Identify and maintain an up-to-date directory of resources within patients' local communities to address individual social determinants of health. Connect patients with the appropriate services to support health and wellness.
• Identify transportation resources and assist with coordination for patients to optimize appointment attendance.
• Assist patients with determining eligibility for Medicaid coverage and submission of required documentation.
• Identify and problem-solve patient obstacles for compliance with an identified treatment plan or patient-directed care goals.
• Engage regularly with care teams to better understand patient needs and how to best assist with their non-clinical needs.
• May complete unit/department-specific duties as outlined in department documents.

REQUIRED QUALIFICATIONS

• High school diploma or an equivalent combination of education and experience from which comparable knowledge, skills, and abilities can be acquired.
• Community Health Worker (CHW) certification by the Missouri Credentialing Board (MCB) within one (1) year as a condition of continued employment in this job classification.
• Two (2) years of experience in a healthcare setting.

PREFERRED QUALIFICATIONS

• Associate's degree (or equivalent credit hours) in a health-related field.
• Experience working with patients to address social determinants of health.
• Additional license/certification requirements as determined by the hiring department.
