Overview

Copley Hospital, located in Morrisville/Stowe VT, is looking for a Community Health Integration Manager to join the team! The Community Health Integration manager is a member of the Quality Department to support the development and implementation of strategies that advance population health goals through management of the Blueprint for Health Program. This role serves as the primary local contact for Blueprint for Health initiatives in the Morrisville Health Service Area (HSA) and is responsible for managing the programmatic and administrative components of the grant agreements. This position is Full-Time (PT available). Copley Hospital has a variety of shifts available across Full-Time, Part-Time, and Per Diem needs.

Compensation

Compensation: $93,600-99,840/year. The posted salary range reflects compensation for candidates with limited experience. An offer may vary based on factors such as experience, education, skills, internal equity, and market data. An offer of employment may be outside of the posted range based on those factors.

Responsibilities
  • Manage Blueprint for Health programs in partnership with the Blueprint Quality Improvement Facilitator, including:
  • Pregnancy Intention Initiative
  • Community Health Team
  • Medication for Opioid Use Disorder
  • Mental Health Gap Closure
  • Ensure compliance with grant requirements and deliverables, including:
  • Preparing and submitting reports and invoices to the state.
  • Collecting, reviewing, and processing invoices from local Blueprint-participating practices.
  • Monitoring and tracking program-related expenses, including training, travel, and other approved costs.
  • Participate in statewide Blueprint for Health meetings, workgroups, and activities both regularly scheduled and as needed.
  • Support the local Community Collaborative through meeting facilitation, monitoring progress on identified priority initiatives, reporting progress to Blueprint and partner organizations, and ensuring effective communication between meetings.
  • Develop and manage community partnerships to increase collaboration between primary care practices, the hospital, and community-based organizations by:
  • Advancing Community Collaborative initiatives.
  • Communicating Community Health Team services and population health programs to community partners.
  • Establishing and maintaining referral pathways and protocols with community partners.
  • Promote alignment across care coordination efforts by serving as a liaison between care teams and community partners, and providing project management support as needed for care coordination efforts in the HSA.
  • Monitor and support practice engagement in Blueprint initiatives, including:
  • Recruit practices to join Blueprint programs.
  • Participation as a Patient-Centered Medical Home (PCMH).
  • Integration of the Community Health Team services.
  • Adoption and implementation of Blueprint care models.
  • Partner with the Quality Department and key stakeholders to develop strategies that support care coordination, align with organizational quality initiatives, and improve health outcomes.
  • Perform other duties as assigned in support of the goals and operations of Copley Hospital.

  • Prior Experience: Five years or more of experience managing projects, teams, and strategic development. Experience working with community partners and leading collaborative work.
