Job summary

Advocate Health is seeking a Community Health Coordinator to implement evidence-based programming across hospital sites in the Winston-Salem area. You will work with community partners to plan events, gather data, and support targeted health initiatives.

The role requires evaluating outcomes, reporting on community benefit, and educating staff about social determinants of health. Some evening/weekend work and travel within the metro area may be needed.

Qualifications

• High school diploma or GED required.
• 2+ years addressing social determinants of health, population health, or related areas.
• Experience designing and evaluating community health programs preferred.
• Ability to summarize program outcomes and benefit data for reporting.

Responsibilities

• Coordinate community health programming at designated hospital sites.
• Attend meetings, coordinate events and screenings, and liaise with community partners.
• Collect and summarize data to monitor program outcomes.
• Support reporting of community benefit data to hospital leadership.
• Engage with community leaders and organizations to address health needs.
• Educate staff and governance on community health issues and interventions.
