Job summary

Jobot is seeking a Family Practice Nurse Practitioner to deliver comprehensive, patient-centered primary care at our Plainfield, NJ site. This full-time, on-site role involves comprehensive assessments, treatment planning, and collaboration with a multidisciplinary team to support community health needs.

Qualified candidates hold MSN or DNP, NJ NP licensure, and FNP board certification, with experience in primary care and underserved populations a plus.

Qualifications

• MSN or DNP degree required.
• Current, unrestricted NP license in New Jersey.
• Board certification as a Family Nurse Practitioner.
• Experience in primary care, family practice or community health preferred; underserved populations a plus.

Responsibilities

• Provide full-scope primary care for patients across the lifespan, including assessments, physical exams, diagnosis and treatment of acute and chronic conditions.
• Order and interpret labs, imaging and other diagnostics to guide clinical decisions.
• Develop evidence-based treatment plans and prescribe medications and preventive care within scope of practice.
• Educate patients and families on prevention, wellness and chronic disease self-management.
• Coordinate care with an interdisciplinary team and keep EMR documentation timely and accurate.
